1. Forms (Microsoft Word documents)

A. Implant Order Form - Used to initiate the design of a TMJ implant for a new patient. B. Informed Consent Form - Completed prior to TMJ implant surgery. C. Device Tracking Form - This form is required only if you did not complete the "Patient Information" section on the Implant Order Form. Device Tracking informaton must be received prior to shipping the implants.

2. Surgical Preparation Manual (Microsoft Word document) - Contains a process overview, CT scan instructions, model preparation instructions, surgical technique, and product insert information.

3. CT Scan Protocol (Microsoft Word document)

4. Post-Op Evaluation Form (Microsoft Word document) - Used only for patients enrolled in clinical study.
