1. Forms

A. Implant Order Form - Used to initiate the design of a TMJ implant for a new patient.

B. Informed Consent Form - Completed prior to TMJ implant surgery.

C. Device Tracking Form - This form is required only if you did not complete the "Patient Information" section on the Implant Order Form. Device Tracking information must be received prior to shipping the implants.

2. Surgical Preparation Manual - Contains a process overview, CT scan instructions, model preparation instructions, surgical technique, and product insert information.

3. CT Scanning Protocol

4. Post-Op Evaluation Form - Used only for patients enrolled in clinical study.
